It is often helpful to our patients to think about the specific concerns before they come in and make a list of questions for the doctor to answer during the examination. If you require x-rays, the receptionist will ask you to come into the office 15 or 30 minutes early so that they may be done before you see the doctor. Routine appointments should be made well in advance. If you are unable to keep you appointment or are going to be late, please call as soon as possible. This courtesy allows us to be of service to other patients.
You must bring your insurance card to every visit at our office. Please be sure to bring your referral if required by your insurer, workers compensation authorization if needed, and a list of medications you presently take.
If another physician has referred you here, please see that he or she sends your records. The physician who referred you will receive a letter discussing your problem and our treatment plan. They will be kept informed of your progress when indicated.
